Communities That Care (CTC) Coordinator 

Job Title: Communities That Care (CTC) Coordinator 
Status: 1.0 FTE 
Location: Hybrid (Applicants must be a MN resident, position based in Chisholm, MN) 
Salary Range: $60,000-$65,000 
Anticipated Start Date: September 1, 2025

Funding Note: This position is currently funded through a multi-year grant and is anticipated to continue through 2030. Continuation is contingent upon ongoing Minnesota Department of Health (MDH) funding and program performance. 

United Way of Northeastern Minnesota (UWNEMN) is seeking a dedicated, community-driven professional to serve as the Communities That Care (CTC) Coordinator. This position plays a pivotal role in advancing upstream prevention efforts supporting youth and families in Hibbing and surrounding communities.

The CTC Coordinator will be responsible for facilitating, organizing, and documenting the implementation of the CTC prevention system – a proven, data-driven approach to reducing youth risk behaviors and promoting well-being. The ideal candidate is a relationship-builder, skilled communicator, and systems thinker who understands the power of prevention and the importance of centering community voice.

This role requires the ability to work across sectors – engaging schools, public health, mental health providers, law enforcement, local government, nonprofits, parents, and youth – to build a shared vision and coordinated action plan. By supporting the work of the Community Board and collaborating closely with CTC Coaches, the Coordinator helps align efforts, interrupt harmful cycles before they begin, and strengthen long-term systems of support that foster resilience and well-being in youth and families.

The CTC Coordinator will report directly to UWNEMN’s Community Impact Director and maintain regular communication with internal staff, board leadership, and external partners to ensure fidelity to the CTC model, alignment with grant objectives, and measurable community impact.

 

Coordination & Facilitation 

  • Provide direct staff support to the Community Board and its workgroups, including facilitation of meetings and follow-up on action items 
  • Collaborate with the CTC Coach and MDH to guide the community through CTC milestones and benchmarks 
  • Coordinate scheduling, logistics, and materials for all CTC Plus workshops, community events, and planning sessions 
  • Prepare for and lead board and workgroup meetings, including agenda development and follow-up communications 
  • Track and delegate workgroup action items to ensure timely progress through the implementation timeline 
  • Maintain regular communication with UWNEMN’s Community Impact Director to ensure alignment with organizational goals, internal initiatives, and reporting deadlines 

Community Engagement & Leadership Support 

  • Build and maintain strong relationships with diverse local stakeholders including school personnel, public agencies, service providers, youth, parents, and government leaders 
  • Partner with the core workgroup to identify and engage key leaders and potential Community Board members 
  • Support onboarding and orientation of new participants using CTC materials and tools
  • Promote a welcoming and inclusive culture within the Community Board that reflects the values of UWNEMN and the CTC system
  • Collaborate with schools and youth-serving organizations to engage young people as active contributors through school-based clubs or Youth Action Boards

Documentation & Reporting 

  • Keep accurate and organized records of all CTC-related activities, including agendas, minutes, attendance, key decisions, and board milestones 
  • Track progress against the official CTC Implementation Timeline and maintain a clear system for organizing milestone and benchmark documentation 
  • Prepare timely narrative and budget reports for funders and internal leadership as needed 
  • Support UWNEMN in grant compliance, budget tracking, and submission of financial documentation as required 
  • Assist in developing materials such as concept papers or reports to support sustainability and funding opportunities 

Communication & Capacity Building 

  • Assist in developing and distributing newsletters, stakeholder updates, flyers, and other outreach materials to raise awareness about CTC efforts 
  • Represent UWNEMN and CTC at community meetings, public forums, and media opportunities as appropriate 
  • Encourage broad-based community participation and visibility of CTC in Northeastern Minnesota 
  • Support and build leadership among youth and adult board members, fostering shared ownership of the prevention process 
  • Attend relevant training, peer learning opportunities, or statewide CTC calls to stay current on prevention best practices and coalition-building strategies 

Required

  • Bachelor’s degree in public health, education, social work, human services, community development, or related field (or equivalent lived/professional experience) 
  • Minimum three years of experience in community-based coordination, youth prevention, or collaborative initiatives • Strong organizational, written, and verbal communication skills 
  • Experience working with cross-sector teams and diverse populations 
  • Ability to work flexible hours, including occasional evenings or weekends 
  • Proficiency in Microsoft Office, Google Workspace, and online meeting platforms (e.g., Zoom, Teams) 
  • Valid driver’s license and access to reliable transportation for travel 

Preferred

  • Familiarity with the Communities That Care prevention model or comparable frameworks 
  • Knowledge of the Iron Range region/its strengths and challenges 
  • Experience with youth engagement models such as Youth Action Boards or student-led groups 
  • Experience in data tracking, grant reporting, or coalition governance
