Thank you for your service, and thank you for your interest in UWNEMN's Veterans Wellness Passport!

By entering your information here, you avoid having to enter the same information for each UWNEMN Veterans Wellness Passport activity. Once UWNEMN receives this form, a staff member will contact you with your personal Passport Number, which you will be able to use to sign up for future Passport activities.

As a reminder, this wellness program is free and open to active duty, veterans, and members of the National Guard or Reserves of all ages, backgrounds, abilities, and experience levels. Service members must live or work in UWNEMN's service territory to be eligible for this program. This event is private to veterans only; all participating veterans must read and agree to the Veterans Wellness Passport waiver

Iron, Nashwauk, Nett Lake, Orr, Palo, Pengilly, Saginaw, Side Lake, Soudan, Tower, Virginia, Winton, and Zim

Unfortunately, this program is only for active duty, veterans, and members of the National Guard and Reserves who work or live in our service territory.
